Creating Connection, Collaboration and Consistency

TLC Support WA is dedicated to providing, researching, and collaborating with quality service providers, to ensure your providers are meeting your needs toward your goals and ensuring we review them regularly. TLC Support WA takes a two-pronged approach to maintain contact, connection, and consistency in your service. 1. Is to connect you with a Client Empowerment Officer, this person is assigned to firstly get to know you, your goals, and your needs and help you through the developing and implementing your plan. 2. You and your Client Empowerment Officer are linked to the Support Coordinator who then supports you in all the following tasks when required. SUPPORT COORDINATION and PYSCHOSOCIAL RECOVERY TASKS -Assist with matters of skill building. -Implement and manage the NDIS plan. -Link the participant to mainstream services as required including mental health, health, and sporting and leisure activities as appropriate. -Solicit therapy reports as required and convey any AT or additional therapy needs to the planner. -Assist with the purchase of any required AT. -Assist with consideration of purchase versus rental of required equipment. -Provide progress reports to the NDIS planner for review. -Provide monitoring updates to the NDIS planner at the frequency requested. -Ensure BSP is completed, updated, and maintained as required. -Assist with matters of employment. -Complete and provide incident reports to the NDIS as required.             We are obligated to provide quality services to our clients. As Support Coordinators we are enforcing safeguards and measures to protect our clients. As a provider all the documentation should be in place and easy to access.
